Taylor McGregor of Marquee Sports Network confirmed the procedure was the full Tommy John operation, not the shorter-recovery internal brace option.
That distinction matters: the internal brace approach typically carries a 6 to 9-month timeline and could have offered a late-2026 return, but the full UCL reconstruction eliminates that possibility, Bleacher Nation reported.
Horton had begun 2026 strong, posting a 2.45 ERA across two starts before exiting an April 4 game with right forearm tightness. He was placed on the 15-day injured list after an MRI that ESPN described as “not clean,” and he later consulted Tommy John specialist Dr. Keith Meister.
The Cubs had been cautiously managing Horton’s innings after his standout 2025 rookie season (11-4, 2.67 ERA in 22 starts), and the organization now targets a 2027 return, with timing still to be determined according to Sporting News. Whether he returns in April or August of that year remains to be seen.
